By booking a Travel vaccination appointment, all patients provide consent to treatment by DocHQ registered Medical professionals or third-party providers.
THESE TERMS AND CONDITIONS ARE SOLELY FOR THE VACCINATION ADVICE AND BOOKING SERVICE OFFERED BY DOCHQ. FOR ANY OTHER SERVICE OFFERED TO USERS, USER WILL HAVE TO ACCEPT DIFFERENT TERMS AND CONDITIONS.
PLEASE NOTE THAT THESE TERMS AND CONDITIONS COVER ALL USE OF OUR SERVICES AS SET OUT IN OUR WEBSITE AND WE RESERVE THE RIGHT TO AMEND, MODIFY AND/OR CHANGE THE SAME FROM TIME TO TIME. FOR THE AVOIDANCE OF DOUBT, THE TERMS APPLICABLE TO THE USER SHALL (SAVE AND EXCEPT ANY CHANGES IMPOSED BY LAW) BE THOSE TERMS AS VALID AT THE TIME OF ANY RELEVANT TRANSACTION.
"Conditions" these terms and conditions as amended from time to time by DocHQ.
"Contract" the contract between the Customer and DocHQ for the supply of a Travel Health Consultation Booking.
"Customer" any UK resident over the age of 16 years of age or legal guardian of a UK resident under the age of 16 who seeks a Travel Health Booking in order to obtain Travel Health Advice and medication for their individual trip.
"Health Care Professional" registered nurse professional, GP or other relevant qualified medical professional who is adequately trained and knowledgeable in providing Travel Health Advice to a Customer.
"Intellectual Property Rights" all patents, rights to inventions, utility models, copyright and related rights, trade marks, service marks, trade, business and domain names, rights in trade dress or get-up, rights in goodwill or to sue for passing off, unfair competition rights, rights in designs, rights in computer software, database right, topography rights, moral rights, rights in confidential information (including know-how and trade secrets) and any other intellectual property rights, in each case whether registered or unregistered and including all applications for and renewals or extensions of such rights, and all similar or equivalent rights or forms of protection in any part of the world.

"Booking Summary" a health risk assessment conducted through the DOCHQ online process for the Customer’s trip.
"Travel Health Booking" an online booking made on DOCHQ website for a face to face Travel Health Consultation with a Health Care Professional, a Booking Summary, is generated and sent to the Health Care Professional who will conduct the face to face consultation.
"Travel Health Consultation" a report provided by DOCHQ or partner CQC approved clinics in the UK, detailing the recommended vaccinations needed for a Customer and detailing a schedule of dates when and which travel vaccinations the Customer has jointly agreed with the Health Care Professional should take place before the Customers trip. Travel Health Consultation is a face to face consultation at a time and clinic of the customers choice.
"Website" shall mean the websites dochq.co.uk, DocHQ App, and any iFrames or APIs developed in relation to health advice created by or on behalf of DocHQ.
"CQC approved Clinics" means all third party partner clinics. Our partner clinics are registered with and comply to the standards required by the Care Quality Commision. https://www.cqc.org.uk/
1.2.1 a person includes a natural person,corporate or unincorporated div class="toc" (whether or not having separate legal personality);
1.2.2 a referenceto a party includes its personal representatives, successors or permitted assigns;
1.2.3 a reference to a statue or statutory provision is a referenceto such statute or statutory provision as amended or re-enacted. A reference to astatute or statutory provision includes any subordinate legislation made under that statue or statutory provision, as amended or re-enacted;
1.2.4 a reference to writing or written includes faxes and e-mails.
2.1 The Contract constitutes the entire agreement between DocHQ and the Customer and any variation to these conditions shall have no effect unless expressly agreed in writing by DocHQ.
2.2 These Conditions apply to the Contract to the exclusion of any other terms that the Customer seeks to impose or incorporate, or which are implied by trade,custom, practice orcourse of dealing.
3.1.1 access to booking a Travel Health Consultation on the Website.
3.1.2 a Booking schedule with one its partners clinics for a Travel Health Consultation appointment.
4.1.1 any information s/he provides in completing the Travel Health Booking questions and in order to create the Travel Health Plan are fully complete and accurate;
4.1.2 s/he will provide DocHQ with such information and materials as DocHQ may reasonably require in order to supply the Booking Summary and Travel Health Plan;
4.1.3 the information s/he provides is for herself/himself and not a third person and the Customer is using the Travel Heath Brief and (if applicable) the Travel Health Plan for his/her own purposes; unless they are providing information for their child, in that case we will consider that they are aware of all their allergies and needs; and
4.1.4 s/he will not pass the Booking Summary and (if applicable) the Travel Health Plan on to any other person as such information is particular to the Customer only and cannot be relied upon by any third party.
8.1.1 death or personal injury caused by its negligence, or the negligence of its employees, agents or subcontractors;
8.1.2 fraud or fraudulent misrepresentation; or
8.1.3 breach of the terms implied by section 2 of the Supply of Goods and Services Act 1982 (title and quiet possesions).
8.2.1 DocHQ shall not be liable to the Customer, whether in contract, tort (including negligence), breach of statutory duty, or otherwise, for any loss of profit, or any indirect or consequential loss arising under or in connection with the contract.
8.2.2 DocHQ's total liability to the Customer in respect of all other losses arising under or in connection with the Contract, whether in contract, tort (including negligence), breach of statutory duty, or otherwise, a deliberate breach of the Contract by DocHQ, its employees, agents or subcontractors shall not exceed £1,000,000 (one million pounds).
